Part One: About the Performance Study

The programme will involve you making changes to what you do, to strengthen your mental fitness and enhance your mood. It will include keeping a diary and reinstating activities you enjoy which you may have stopped doing, helping you get back on track when you get off course.
You will be assigned a Performance Habits Coach (PHC) who you will speak with about 6-8 times over a few months to offer guidance and support. Both before and after the programme you will complete several short questionnaires to measure your wellbeing, to see if the intervention has affected how you feel.
PART TWO: How will we use information about you?
We will need to use information from you to carry out this research study. This information will include some personal information (such as your name and contact details) along with information about your mental and physical wellbeing.
People who do not need to know who you are will not be able to see your name or contact details. Your data will have a code number instead.
We will keep all information about you safe and secure.
Once we have finished the study, we will keep some of the data so we can check the results. We will write our reports in a way that no-one can work out that you took part in the study.
- You can stop being part of the study at any time, without giving a reason, but we, at the University of York will keep information about you that we already have.
- We need to manage your records in specific ways for the research to be reliable. This means that we won’t be able to let you see or change the data we hold about you.

The only time we would break our duty of confidentiality (which may include us having to contact your GP) is if we are worried that you – or someone else – was being, or was likely to be, harmed. If that happens, we will talk with you about it.
- Researchers from the University of York will analyse the data collected.
- The results of the study will be published in a report and in academic journals.
- An easy read version of the main report will be produced and shared with participants via email or post, depending upon their preference.
